Appropriates $1,250,000 in fiscal year 2024 and $1,250,000 in fiscal year 2025 from the general fund to the commissioner of health for a grant to Breakwater Health Network.

Grant funds must be used to identify a new electronic health record (EHR) system and contract with a vendor for purchase, testing, and implementation.

Selected EHR system must improve patient care and documentation, enable patient information exchange with other treating providers, and establish an equity framework for collecting social determinants of health information.

New EHR system must improve patient and provider experience, population health outcomes, reduce healthcare costs, and strengthen security of patient health information.

Designated as a onetime appropriation.
