Commissioner of human services must develop a county-administered rural medical assistance (CARMA) model in collaboration with the Association of Minnesota Counties and county-based purchasing plans.

CARMA model must provide a county-owned alternative to prepaid medical assistance programs while integrating health care and social services to address social determinants of health in rural communities.

Model must account for smaller enrollee populations and limited local providers in rural areas, including behavioral health, oral health, specialty care, and nonemergency medical transportation services.

Rural counties have the option to apply and participate in CARMA as an alternative to the prepaid medical assistance program, with the commissioner determining eligibility and participation terms.

Commissioner must report recommendations and implementation plan to legislative health care policy and finance committees by January 15, 2025, addressing prior litigation recommendations and identifying necessary federal waivers and draft legislation.
