Removes the 12-month incarceration limit for medical assistance suspension, suspending coverage for all incarcerated individuals regardless of sentence length until release.

Requires the commissioner of human services to provide outreach, education, and enrollment support to incarcerated individuals and ensure all eligible incarcerated individuals are enrolled in medical assistance no later than 45 days before their expected release.

Establishes medical assistance coverage for care transition services during the 30 days prior to release for incarcerated individuals with chronic physical or behavioral health conditions at high risk of poor health outcomes, including assessments, screenings, treatment, case management, and a 30-day supply of prescribed medications upon release.

Care transition services include medication-assisted treatment for substance use disorders, behavioral health services, family planning services, and case management with appointment scheduling assistance.

Directs the commissioner of human services to apply for a federal Medicaid Reentry Section 1115 Demonstration Opportunity waiver and consult with the commissioner of corrections, sheriffs, lead agencies, and individuals with lived experience of incarceration in preparing the application; effective 90 days after federal approval.
