Requires social services agencies to provide parents or guardians with a one- to two-page summary of the out-of-home placement plan using a form developed by the commissioner before the plan is signed.

The summary must clearly summarize the plan's contents and list requirements and responsibilities for parents or guardians using plain language.

The summary must be updated and provided to parents or guardians whenever the out-of-home placement plan is updated.

Applies to all children placed in foster care by court order or voluntary placement agreement under Minnesota Statutes sections 260C.227 or chapter 260D.

Effective date: March 1, 2025.
