Establishes a one-time grant program administered by the commissioner of human services to incentivize home and community-based service providers to help individuals move from congregate living settings into their own homes.

Eligible grant recipients are providers of home and community-based services under Minnesota Statutes, chapter 245D, who must apply using forms and timelines established by the commissioner.

Allowable uses of grant funds include enhancing staffing to support housing options, covering moving expenses, implementing innovative approaches to support individuals with disabilities, providing financial incentives for successful transitions, and other commissioner-approved activities.

Appropriates $2,500,000 from the general fund in fiscal year 2025, with a general fund base of $2,500,000 in fiscal year 2026 and $0 in fiscal year 2027.

Grant program expires June 30, 2026.
