• License holders must complete preliminary support plan addendums within 15 days of service initiation and comprehensive assessments in health management, personal safety, and symptom management within 45 days of service or 60 calendar days, whichever is shorter.

• Initial planning meetings must be held before 45 days of service or within 60 calendar days to determine service scope, desired outcomes, service preferences, integrated settings, skill development opportunities, community participation, relationships, employment opportunities, and provider coordination.

• License holders must develop service plans within 10 working days of the initial planning meeting documenting supports, methods, measurable outcomes, implementation dates, and responsible staff, with signatures from the person and case manager required within 20 working days.

• Designated coordinators must have a baccalaureate degree plus one year of direct care experience, or equivalent combinations of associate/diploma degrees with additional experience, and must provide supervision, staff instruction, and outcome evaluation.

• Designated managers must meet the same education requirements as coordinators plus have a minimum of three years of supervisory-level experience in programs serving persons with disabilities or elderly individuals.
