Appropriates $60,000,000 in fiscal year 2025 from the general fund to the commissioner of human services for grants to five continuums of care (three metropolitan area and two from outside the seven-county metropolitan area).

Grants must be used to fill gaps in homelessness prevention programs including crisis response services, shelters and outreach, diversion, unsheltered navigation services, and housing-focused case management.

Commissioner must consider Department of Housing and Urban Development competitive scores when selecting the two nonmetropolitan continuums and may distribute funds through hosting nonprofits if requested.

Appropriation must be divided equally among all five continuums based on the same amount per person experiencing homelessness according to the most recent point-in-time count, with up to 10 percent available for administrative expenses.

Funding base is set at $60,000,000 for fiscal years 2026, 2027, 2028, and 2029, reducing to $0 in fiscal year 2030.
