Appropriates $1,040,000 in fiscal year 2025 from the general fund to the commissioner of human services for a competitive grant to a home care nursing provider for a pediatric hospital-to-home transition pilot program.

Pilot program must be developed in coordination with Fairview Masonic Children's Hospital, Gillette Children's Specialty Healthcare, and Children's Minnesota of St. Paul and Minneapolis to serve medical assistance patients, including those under community alternative care, community access for disability inclusion, and developmental disabilities waivers.

Grant funds must support administrative, training, and auxiliary services to reduce delayed discharge days, avoidable rehospitalizations, unnecessary emergency department use, long-term nursing needs, and missed school days for complex pediatric patients.

Grant funds cannot be used to supplement payment rates for services under Minnesota Statutes chapter 256B, and the appropriation is available until June 30, 2027.

Commissioner must submit a report by December 15, 2026 to legislative committees with jurisdiction over health and human services documenting eliminated delayed discharge days, rehospitalizations, emergency department admissions, missed school days, and return on investment estimates.
