Establishes a statewide Provider Orders for Life-Sustaining Treatment (POLST) program to be administered by the commissioner of health, with implementation required by January 1, 2025.

Creates a statewide POLST registry accessible 24/7 through web portal, telephone/radio call center, and electronic health records for emergency medical responders, health care facilities, and other authorized users to access patient treatment preferences.

Permits patients or legal representatives to revoke POLSTs, remove them from the registry, or choose not to include them; establishes that participation is voluntary for patients and providers.

Classifies all POLST registry data identifying patients, providers, or facilities as private data, exempt from subpoena and civil discovery, while providing immunity for good faith reporting or actions based on registry information.

Appropriates $500,000 in fiscal year 2025 for registry establishment and administration, with $400,000 base funding for fiscal years 2026-2027; appropriates $100,000 in fiscal year 2025 for education and training contract with Minnesota POLST Steering Committee.
