Requires the commissioner's state medical review team to review all medical evidence and assist applicants in obtaining missing evidence before denying disability determinations due to insufficient evidence.

Requires appeals of disability determinations to be decided within specified timelines, with appeals not decided timely automatically reviewed by the chief human services judge.

Allows licensed independent clinical social workers, licensed graduate social workers, and licensed independent social workers to refer individuals for disability determinations regardless of county or Tribal agency employment.

Requires lead agencies to provide waiver participants with written explanations of how their consumer-directed community supports budget was calculated, a copy of the budget formula, information about traditional waiver service alternatives, and information about appeal rights.

Directs the commissioner by December 31, 2024 to seek waiver plan changes including eliminating a 0.7 base rate adjustment factor, clarifying allowable goods and services, and permitting enhanced rates for personal assistance services and family caregivers when assessed needs justify them.
