Commissioner of health shall develop recommendations for a statewide registry of POLST forms to ensure patient medical treatment preferences are followed by all health care providers.

Registry must allow submission of completed POLST forms and timely access by health care providers and emergency medical service personnel.

Commissioner shall develop recommendations addressing electronic capture and storage, information security, confidentiality protections, access limits, registry location, and ongoing funding models.

Commissioner shall establish an advisory committee with representatives from physicians, physician assistants, advanced practice registered nurses, nursing homes, emergency medical systems, hospice and palliative care providers, disability community, attorneys, medical ethicists, and religious community.

Commissioner must submit recommendations to legislative committees with jurisdiction over health and human services policy and finance by February 1, 2024, with registry implementation no later than December 1, 2024.
