• Commissioner of Health shall establish a single grant to expand the community care hub model in Minnesota by organizing health and social care service providers to address health-related social needs such as food insecurity, housing instability, and lack of transportation access.

• Grant applicants must be federally recognized as selected community care hubs by the Administration for Community Living and Centers for Disease Control and Prevention, hold contracts with Minnesota health plans, and demonstrate active community engagement in providing health and social care services.

• Grantee must use funding to engage community organizations, expand social care service reach, centralize administrative functions (contracting, payment operations, referral management, quality improvement, technology, data collection), and create sustainable financial pathways for services statewide.

• Grantee must report community care hub initiative outcomes to the Commissioner of Health on forms and timelines established by the Commissioner, and the Commissioner shall evaluate the initiative using measures for cost savings, impact, and health outcomes.

• $8,900,000 appropriated in fiscal year 2025 for the community care hub grant and $1,000,000 for evaluation and technical assistance, both available until June 30, 2029; effective July 1, 2024.
