Adds Alzheimer's disease and other forms of dementia to the list of priority health areas in Minnesota's health disparities reduction program.

Expands the community grant program to include projects and initiatives addressing racial and ethnic disparities in morbidity and mortality rates from Alzheimer's disease and dementia.

Allows up to 20 percent of available funds to be awarded as planning grants for community assessment, priority determination, and strategy development.

Eligible grant applicants include faith-based organizations, social service organizations, community nonprofits, community health boards, and community clinics.

Appropriates unspecified amounts from the general fund to the commissioner of health for fiscal years 2025 and 2026 to support grants under this program.
