Commissioner of health must include dates and types of immunization administered within two years preceding death on each death certificate.

Death certificate immunization records must document eight specific data elements: vaccination administration date, vaccine dose volume and units, expiration date, lot number, product name, route of administration, and site of administration.

Commissioner of health shall produce public reports within ten days of the state fiscal year beginning detailing total deaths occurring within 24 hours, three days, one week, three weeks, ten weeks, 25 weeks, and one year of immunization, broken down by vaccine type, gender, age group, race, ethnicity, and comorbidity type.

Commissioner of health must conduct a biennial cause of death and injury study and issue a public report within two weeks before each general presidential election and general midterm or gubernatorial election, including deaths with contributing conditions and injuries noted in physician records with demographic data.

Commissioner of health shall select four independent public health auditors biennially from organizations not affiliated with government or pharmaceutical industry to review and audit the immunization death data reports.
