Establishes a grant program administered by the commissioner to provide ongoing funding for projects and services serving older adults and people with dementia who no longer meet criteria for existing medical assistance programs

Targets new and expanded home and community-based services with proven outcomes to build statewide capacity and stability among community providers

Includes family caregiver supports for seniors not eligible for medical assistance

Amends Minnesota Statutes 2022, section 256B.0917, by adding subdivision 15
