Modifies Minnesota Statutes section 256B.439 to specify demographic information the commissioner may request from long-term care providers for consumer surveys, including age, race, ethnicity, and gender identity.

Requires providers to furnish available contact and demographic information to the commissioner and notify clients and key representatives that their information has been shared.

Mandates the commissioner develop and administer resident experience surveys for assisted living facility residents and family surveys for families of assisted living facility residents.

Requires assisted living facilities licensed under chapter 144G to participate in surveys when requested by the commissioner.

Makes appropriated money for administering surveys available in either fiscal year of the biennium in which it is appropriated.
