Adds critical access nursing facility rate adjustment as a component of the external fixed costs payment rate for nursing facilities under Minnesota Statutes 256R.25.

Authorizes the commissioner to designate nursing facilities as critical access nursing facilities on a competitive basis to preserve access in isolated areas, rebalance long-term care, and improve quality.

Allows supplemental payments above operating payment rates for designated critical access facilities, approved through memorandum of understanding and included as time-limited rate adjustments in the external fixed payment rate.

Requires memorandum of understanding to specify that critical access nursing facility designation must be removed if the facility undergoes a change of ownership.

Designations are limited to a maximum period of two years, after which benefits are removed, though facilities may apply for continued designation.
