Suspends medical assistance eligibility for incarcerated individuals during incarceration and requires reinstatement without reapplication upon release, regardless of sentence length.

Requires the commissioner to provide outreach, education, and enrollment support to incarcerated individuals and ensure all eligible individuals are enrolled no later than 45 days before expected release.

Establishes medical assistance coverage for care transition services during the 30 days prior to release for incarcerated individuals with chronic physical or behavioral health conditions at high risk of poor health outcomes.

Care transition services include comprehensive health assessments, infectious disease screening and treatment, behavioral health services, medication-assisted treatment, case management, 30-day prescription medication supply, and family planning services.

Directs the commissioner of human services to apply for a federal Medicaid Reentry Section 1115 Demonstration Opportunity waiver to implement these provisions, in consultation with the corrections commissioner, sheriffs, and formerly incarcerated individuals.
