Adds Alzheimer's disease and other forms of dementia to the list of priority areas for Minnesota's health disparities reduction program that targets American Indians and populations of color.

Expands the community grant program under Minnesota Statutes section 145.928 to include grants for projects reducing racial and ethnic disparities in morbidity and mortality rates from Alzheimer's disease or other forms of dementia.

Maintains existing priority grant criteria including community support, research-based strategies, complementary activities, racially and ethnically appropriate approaches, and implementation through community-based organizations reflecting the target population.

Appropriates unspecified amounts from the general fund to the Commissioner of Health for fiscal years 2025 and 2026 to support grants under the expanded program.
