Establishes a onetime grant program administered by the commissioner of human services to incentivize providers to help individuals move from congregate living settings into their own homes.

Eligible recipients are providers of home and community-based services under Minnesota Statutes, chapter 245D, who must apply to the commissioner using forms and timelines established by the commissioner.

Grant funds may be used for enhancing staffing and resources, housing expenses for moves, implementing innovative support approaches, financial incentives for successful transitions, and other commissioner-approved activities.

Appropriates $2,500,000 from the general fund in fiscal year 2025, with a general fund base of $2,500,000 in fiscal year 2026 and $0 in fiscal year 2027.

Program expires June 30, 2026.
