Appropriates $1,040,000 in fiscal year 2025 from the general fund to the commissioner of human services for a single competitive grant to a home care nursing provider to establish a pilot program.

Pilot program will coordinate with Fairview Masonic Children's Hospital, Gillette Children's Specialty Healthcare, and Children's Minnesota to expedite pediatric hospital-to-home discharges for patients on medical assistance, including community alternative care waiver, community access for disability inclusion waiver, and developmental disabilities waiver.

Grant funds must support administrative, training, and auxiliary services to reduce delayed discharge days, avoidable rehospitalization days, unnecessary emergency department utilization, long-term nursing needs, and school days missed by pediatric patients.

Grant money cannot be used to supplement payment rates for services covered under Minnesota Statutes, chapter 256B, and is available until June 30, 2027.

Commissioner must submit a report by December 15, 2026 to legislative committees with jurisdiction over health and human services measuring delayed discharge days eliminated, rehospitalization days eliminated, unnecessary emergency department admissions eliminated, missed school days eliminated, and return on investment.
