Commissioner of human services shall authorize one or more risk-based demonstration partnerships between lead agencies and medical assistance providers to serve at least 500 people enrolled in medical assistance waivers under specified statutes.

Demonstration partnerships will evaluate informed choice and expedited service authorizations for technology-related waiver benefits including environmental accessibility adaptations, specialized supplies and equipment, and client and caregiver training in the home.

Lead agencies must identify and prioritize eligible individuals based on criteria including significant clinical acuity, multiple emergency room visits or inpatient admissions in prior 365 days, behavioral or complex chronic conditions, transportation challenges, or inability to find available primary care providers.

Lead agencies shall provide expedited review and approval for service authorizations, evidence-based cost data and quality analysis to the commissioner, and collect stakeholder feedback from recipients, family caregivers, and partners.

Commissioner must approve one or more demonstration partnerships by October 1, 2024, for services beginning January 1, 2025, in at least five counties, and may report evaluation outcomes and expansion recommendations to legislative committees.
