Amends behavioral health home services provider requirements to include screening for substance use disorder and alcohol/tobacco use, with referrals to appropriate resources and tracking of referral outcomes.

Expands behavioral health home services delivery standards to require team-based care coordination, person-centered planning, diagnostic assessments within six months, health coaching, and discharge planning protocols.

Increases general assistance standard of assistance to $350 per month effective October 1, 2024, with annual adjustments based on consumer price index changes beginning October 1, 2025.

Changes housing support terminology from "licensed and registered" to "authorized" settings and modifies cost-neutral transfer provisions for emergency shelter beds and housing support funds.

Repeals sections 256D.19, 256D.20, and 256D.23 relating to township poor relief systems and temporary county assistance programs, with renumbering of section 256D.21 to 261.004.
