Prohibits private agencies from providing case management services to home and community-based waiver recipients if the agency has any financial interest in providing other services included in the recipient's support plan.

Defines "private agency" as any agency not identified as a lead agency under Minnesota Statutes section 256B.0911, subdivision 10.

Requires case management services to be provided by public or private agencies enrolled as medical assistance providers that meet all federal waiver plan requirements.

Maintains case manager responsibilities for professional judgment activities including finalizing person-centered support plans, ongoing assessment and monitoring of needs, and adjustments to support plans.

Requires the Department of Human Services to provide case managers with no less than 10 hours of case management education and disability-related training annually, including person-centered planning instruction.
