Expands conscience-based immunization exemptions by allowing students to submit a notarized statement of conscientiously held beliefs to opt out of any immunization mandated by the institution, not just those in subdivision 2.

Permits students exempt from immunizations to remain enrolled and experience no disruption in their academic term.

Requires institutions that mandate immunizations to provide conspicuous notice to students informing them of available exemptions.

Maintains existing medical exemptions: physician-signed statements showing medical reasons, prior natural disease experience, or laboratory-confirmed immunity.

Requires institutions to forward conscience-based exemption statements to the Minnesota commissioner of health.
