Correction orders and conditional licenses for home and community-based services (Chapter 245D) providers must be specific to the service site where violations occurred, not automatically applied to other sites or licenses held by the same provider

Conditional licenses may only be issued to Chapter 245D providers if they fail to implement or follow requirements of the most recent correction order, and the commissioner must provide advance warning that the next audit could result in a conditional license

Commissioner may reduce conditional license duration if providers demonstrate compliance or progress, and must refer providers to legal assistance or technical assistance resources when subject to enforcement actions

Commissioner must partner with culturally specific organizations to provide education on licensing requirements and offer alternative dispute resolution services when providers appeal correction orders

Annual report to legislature required beginning January 1, 2026, detailing correction orders and conditional licenses issued to Chapter 245D providers, including data on zip codes, employee counts, license effective dates, and collaborative safety improvement efforts
