Prohibits health care providers from charging facility fees for nonemergency services at provider-based clinics (off-campus facilities located at least 250 yards from main hospital buildings), including telehealth services

Bans facility fees for outpatient evaluation and management services regardless of where they are provided, with authority for the Commissioner of Health to add other services to the prohibition

Requires hospitals and health systems to submit annual reports to the Commissioner of Health by January 15, 2027 detailing facility fees charged, billed, and collected, including breakdowns by facility location and top revenue-generating procedures

Establishes enforcement mechanisms including classification as an unlawful business practice, administrative penalties up to $1,000 per occurrence, and audit authority with four-year record retention requirements

Repeals existing Minnesota Statutes section 62J.824, which only required disclosure of facility fees to patients rather than prohibiting them
