Amends Minnesota Statutes section 62Q.78 to simplify the definition of "covered services" for dental care to mean services that are reimbursed in whole or in part under a dental provider agreement

Removes previous language that defined covered services to include services subject to contractual limitations like deductibles, co-payments, coinsurance, waiting periods, and annual maximums

Prohibits dentists from charging more than their usual and customary rate for services or procedures that are not covered services

Requires dentists to provide patients with a treatment plan including anticipated services and estimated costs before performing non-covered services

Preserves dental plans' ability to apply balance billing restrictions, waiting periods, frequency limitations, deductibles, and maximum annual benefits to covered services
