Establishes the Program of All-Inclusive Care for the Elderly (PACE) as a formal service delivery system in Minnesota, authorized under the Social Security Act, to provide integrated medical and long-term care services to eligible persons age 55 and older who meet nursing facility level of care criteria.

Requires PACE organizations to be not-for-profit entities organized under section 501(c)(3), operate under agreements with both CMS and the state, and use risk-based capitated financing where they assume full responsibility for all enrollee costs.

Sets implementation timeline: state plan amendment to CMS by October 1, 2025; competitive bidding process by December 31, 2025; PACE organizations establish operations by June 30, 2026; services begin January 1, 2027.

Removes existing PACE authorization language from section 256B.69 and creates new standalone section 256B.6902 with detailed requirements for contracts, enrollment/disenrollment, payment rates, and commissioner duties including data collection and rate methodology development.

Enrollment in PACE is voluntary and enrollees may disenroll at any time; however, PACE enrollees become ineligible for payment through other Medicare, medical assistance, or MinnesotaCare programs while enrolled.
