Provides up to $100,000 per eligible school for one year of planning activities and up to $200,000 annually per school for up to three years of implementation

Modifies eligibility criteria to include schools on continuous improvement plans or in districts with approved achievement and integration plans, with priority given to schools with high free/reduced-price meal populations, homeless/mobile students, and demonstrated success

Requires school leadership teams of at least 12 members (including parents, teachers, principal, and community representatives) to meet quarterly and oversee baseline analyses, community school plans, and programming implementation

Expands definition of "community organization" to include community centers, universities, and nonprofits focused on specific geographic areas, and clarifies that consortiums require at least one school partnered with at least one community organization
