Establishes a Commissioner of Direct Care and Treatment as a cabinet-level position appointed by the governor, replacing the nine-member executive board governance structure created in 2024

Dissolves the Direct Care and Treatment executive board upon enactment, with authority temporarily transferring to the Commissioner of Human Services until July 1, 2025, when the new commissioner assumes full control

Formally designates the Department of Direct Care and Treatment as a state department, giving it the same status as other major agencies like the Department of Human Services and Department of Corrections

Transfers custody of all civilly committed persons under chapters 253B and 253D, along with management of state hospitals for persons with developmental disabilities, mental illness, or substance use disorders, to the new commissioner effective July 1, 2025

Requires the governor to appoint the initial commissioner by July 1, 2025, and establishes a stakeholder advisory committee (expiring December 31, 2027) including legislators, county representatives, union members, and mental health advocates
