Requires the Commissioner of Human Services to submit the Waiver Reimagine plan to a new Legislative Task Force, the legislature, and all waiver service recipients at least six months before seeking federal approval, and prohibits federal submission without task force approval

Establishes a 26+ member Legislative Task Force on Waiver Reimagine including legislators, waiver recipients, family members, providers, county employees, disability advocates, and state agency representatives, with appointments due by September 30, 2025

Mandates individual budgets be based on assessed needs rather than location of services, including additional resources necessary for individuals to live in the least restrictive environment

Task force must examine consolidation of four disability home and community-based waiver programs into two programs, budget methodologies, rate exceptions, the MnCHOICES 2.0 assessment tool, and intended/unintended consequences of Waiver Reimagine

Task force must submit its report by January 15, 2027, and expires upon report submission or December 31, 2027, whichever is earlier
