Commissioner of Health must include on each death certificate all immunizations administered within 2 years preceding death, documenting administration date, dose volume, lot number, product name, route, and site of administration

For decedents under 18 years old, the complete immunization record must be attached as a text field to the death certificate

Commissioner must produce annual public reports within 10 days of the state fiscal year start showing deaths that occurred within 24 hours, 3 days, 1 week, 3 weeks, 10 weeks, 25 weeks, and 1 year of immunization, broken down by vaccine type, gender, age, race, ethnicity, and comorbidity

Biennial cause of death and injury study required, with public report issued 2 weeks before the date that is 1 year prior to each presidential and gubernatorial election

Commissioner must select 4 independent auditors biennially to review the data, who cannot be affiliated with government, pharmaceutical industry, or public officials; applies to deaths occurring on or after January 1, 2026
