State policy objectives for incentive payments include resident diversion/discharge to community settings, technology adoption, improved quality scores on the Minnesota Nursing Home Report Card, and reduced acute care costs.

Planned closure rate adjustments are calculated at $2,080 per net reduction of nursing facility beds, with adjustments becoming effective on January 1 or July 1 following facility closure completion.

Facilities may reassign planned closure rate adjustments to another facility under the same ownership within three years of the effective date.
