Allows lead agencies to conduct annual abbreviated assessments instead of full reassessments for waiver recipients ages 22-64 with no significant changes in function or needs, using a commissioner-approved tool

Abbreviated assessments must verify current needs, confirm level of care, inform of alternatives, provide choice between institutional or home-based services, screen for changes warranting full reassessment, review service plans, and reauthorize services

Requires informed consent from the person or legal representative to receive an abbreviated assessment; individuals may request a full reassessment at any time

Effective January 1, 2026, or upon federal approval, whichever is later
