Commissioner of Health must establish a grant program for mobile clinics to provide youth sports physicals at Minnesota schools, with grant recipients required to provide at least 25% matching funds from nonstate sources

Youth sports physicals must include cardiac screening, respiratory assessment, blood pressure evaluation, musculoskeletal examination, family medical history review, and mental health screening (covering stress, anxiety, depression, sleep, substance use, and eating disorders)

Mobile clinics must be staffed with at least one physician, physician assistant, or advanced practice registered nurse, and must provide physicals free of charge to uninsured students

Grant recipients must conduct school outreach, provide families with preventive health care information, and report data to the commissioner on physicals provided, schools served, and referrals made

Appropriates $1,000,000 in fiscal year 2026 and $600,000 in fiscal year 2027 from the general fund
