Replaces Minnesota's state-specific network adequacy standards (30 miles/30 minutes for primary care, mental health, and general hospital services) with federal time and distance standards under 45 CFR 155.1050

Repeals the 60 miles/60 minutes maximum travel standard for specialty physician services, ancillary services, specialized hospital services, and other health services

Maintains the $500 waiver application fee for health carriers unable to meet network adequacy requirements, but removes references to the repealed subdivision 3 standards

Preserves the waiver process allowing carriers to use telehealth to address network inadequacy when no providers of a specific type exist in a county

Retains the exemption for referral centers providing highly specialized care such as organ transplants from network adequacy distance requirements
