Establishes a MinnesotaCare public option available to individuals with incomes above 200% of the federal poverty guidelines who meet other MinnesotaCare eligibility requirements, with enrollment through the MNsure website during open or special enrollment periods.

Sets income-based premium contributions ranging from 4.88% of household income (201-250% FPL) to 10% of household income (551% FPL and above), with annual deductibles of $500-$1,500 for those above 400% FPL.

Phases in enrollment limits: 10,000 enrollees above 400% FPL in 2027, 15,000 above 550% FPL in 2028, with no limits starting in 2029.

Requires managed care plans serving public option enrollees to reimburse health care providers at rates equal to or greater than Medicare fee-for-service rates.

Directs the Commissioner of Commerce to submit a Section 1332 federal waiver to obtain pass-through funding equal to premium tax credits and cost-sharing reductions that enrollees would otherwise have received, with most provisions effective January 1, 2027, or upon federal approval.
