Establishes a new "MinnesotaCare Plan" as a qualified health plan offered through MNsure by private health carriers, requiring carriers to offer the plan in every county where they sell individual market coverage

Sets income-based monthly premiums ranging from $4 to $80 for enrollees at or below 200% of federal poverty level, with no premiums for children under 21, individuals below 35% of poverty, and American Indians

Requires plans to maintain 94% actuarial value with cost-sharing exemptions for children under 21 and American Indians, and must cover all health services currently reimbursed under MinnesotaCare

Directs the Commissioner of Commerce to submit a Section 1332 federal waiver requesting 100% of premium tax credit value (instead of the 95% received under the current basic health program) for enrollees up to 250% of federal poverty level

Requires the Commissioner of Human Services to request suspension of Minnesota's basic health program (MinnesotaCare) during the waiver period, while automatically transitioning current MinnesotaCare enrollees to the new plan with no changes to their premiums, benefits, or cost-sharing
