Funds a health awareness hub pilot project targeting health care education and physical and mental wellness needs of elderly individuals within the African immigrant community

Services must include culturally relevant support, resources, and preventive care education from medical practitioners with similar backgrounds to the community served

Referrals must be made to culturally competent programs, supports, and medical care

Organization for Liberians in Minnesota must submit an evaluation of the pilot project to the commissioner within six months of the project's conclusion
