Extends the deadline for the Commissioner of Human Services to submit the pediatric hospital-to-home transition pilot program report from December 15, 2026 to October 15, 2027

The pilot program provides a competitive grant to a home care nursing provider to expedite hospital-to-home discharges for pediatric patients on Medical Assistance, including those under the community alternative care, community access for disability inclusion, and developmental disabilities waivers

Partner hospitals include Fairview Masonic Children's Hospital, Gillette Children's Specialty Healthcare, and Children's Minnesota of St. Paul and Minneapolis

The required report must document outcomes including delayed discharge days eliminated, rehospitalization days eliminated, unnecessary emergency department admissions prevented, missed school days eliminated, and estimated return on investment
