Removes notarization requirement for conscientious belief vaccine exemptions but adds new requirement that parents/guardians complete an educational consultation with a healthcare provider who reviews risks and benefits of immunizations

Requires the Commissioner of Health to develop and make available an immunizations education module covering vaccine risks and benefits, which must be reviewed before obtaining a conscientious belief exemption

Grants the Commissioner of Health authority during disease outbreaks to issue written determinations allowing schools and child care facilities to exclude unvaccinated children with conscientious belief exemptions

Defines "provider" for exemption purposes as a licensed physician, licensed physician assistant, or advanced practice registered nurse

Appropriates an unspecified amount from the general fund in fiscal year 2027 as a one-time appropriation for developing the immunizations education module
