Fertility awareness-based methods defined as tracking menstrual cycles through biological signs (body temperature, cervical fluid, hormone levels) using methods such as symptothermal, Creighton model, and Billings ovulation methods

Reproductive health conditions covered include endometriosis, adenomyosis, polycystic ovary disease, uterine fibroids, blocked fallopian tubes, hormone imbalances, thyroid conditions, and male-factor infertility

Effective date is August 1, 2026
