Commissioner of Health must establish a single grant to expand the community care hub model, which connects health care institutions with community-based organizations addressing health-related social needs like food insecurity, housing instability, and transportation access

Eligible grant applicants must be federally recognized community care hubs by the Administration for Community Living and CDC, hold contracts with Minnesota health plans, and demonstrate active engagement in health and social care coordination

Grant funds must be used to organize community-based organizations, expand social care services, centralize administrative functions (contracting, payments, referrals, technology, data collection), and create sustainable financial pathways statewide

Appropriates $8,900,000 in fiscal year 2026 for the grant and $1,000,000 for evaluation and technical assistance, both available until June 30, 2030

Effective date is July 1, 2025; grantee must report outcomes to the commissioner, who will evaluate cost savings and health impact
