Eliminates medical assistance asset limits and premiums for employed persons with disabilities, including those age 65 and older who were previously enrolled in the MA-EPD program for at least 24 consecutive months

Establishes tiered enhanced payment rates for Community First Services and Supports (CFSS) based on daily service hours: 7.5% for 10-14.9 hours, 12.5% for 15-19.9 hours, and 17.5% for 20+ hours per day

Creates new 25% enhanced rate for CFSS provided by licensed registered nurses, licensed practical nurses, certified home health aides, or certified nursing assistants

Adds 10% enhanced rate components for CFSS provided during weekend hours (Friday 8 p.m. to Monday 8 a.m.) and overnight hours (10 p.m. to 6 a.m.)

Directs the commissioner to seek federal approval to reimburse CFSS during acute care hospital stays to preserve functional abilities and ensure smooth transitions to home and community-based settings, effective January 1, 2026, or upon federal approval
