Correction orders and conditional licenses for home and community-based services (Chapter 245D) providers must be specific to the service site where violations occurred, not applied to other sites unless the commissioner provides articulable basis for doing so

Conditional licenses may only be issued to Chapter 245D providers who fail to implement requirements of the most recent correction order, and the commissioner must warn providers beforehand that future violations will result in a conditional license

Commissioner must refer Chapter 245D providers facing enforcement actions to legal assistance or technical assistance resources upon request, and must partner with culturally specific organizations to provide licensing education and training

Commissioner may reduce conditional license duration if the provider demonstrates compliance or progress toward compliance before the period expires

Annual reporting to the legislature required starting January 1, 2026, on correction orders and conditional licenses issued to Chapter 245D providers, including data on locations, employee counts, license dates, and collaborative safety improvement efforts
