Allows lead agencies to conduct abbreviated annual reassessments for people ages 22-64 receiving disability waiver services who have no significant changes in function or needs, using a commissioner-approved tool

Requires a full (nonabbreviated) reassessment at least once every three years for recipients of developmental disability, community access for disability inclusion, community alternative care, and brain injury waiver services

Permits abbreviated assessments to be conducted remotely via video or telephone, following the same rules as other remote reassessments

Abbreviated assessments must verify current needs, confirm level of care, inform recipients of alternatives, screen for changes warranting full reassessment, review the person-centered services plan, and reauthorize services

Effective January 1, 2026, or upon federal approval, whichever is later
