Requires candidates filing an affidavit of candidacy to include a statement signed under oath by at least two unrelated district residents attesting to personal knowledge that the candidate resides at the address identified

Candidates must present a valid driver's license, state ID card, or election day registration documentation showing their current address of residence when filing; filing officers must reject affidavits where the address and documentation do not match

Removes the option for candidates to use a campaign contact address instead of their actual residence address on the affidavit (except when requesting address privacy classification)

Establishes criminal penalties for willfully making false statements on an affidavit of candidacy: up to 90 days imprisonment, a $10,000 fine, or both

Effective the day following final enactment and applies to affidavits submitted during filing periods beginning on or after that date
