Establishes a demonstration project for provider-led coordinated services organizations to coordinate Medical Assistance and Medicare services for people with disabilities, prioritizing enrollee choice and addressing health-related social needs

Eligible enrollees must be either dual-eligible for Medical Assistance and Medicare receiving waiver services under section 256B.49, or enrolled in Medical Assistance special needs basic care receiving those waiver services

Participating organizations must be nonprofits with shared governance, include home and community-based service providers, have partnerships with managed care plans, and demonstrate capacity for care coordination, population health activities, and community intervention programming

Creates a risk-adjusted per-member, per-month payment system paid quarterly, with potential incentive payments for meeting quality and performance targets, plus a total cost of care risk-gain sharing payment model

Appropriates unspecified amounts for fiscal years 2026 and 2027 for innovation and capacity-building grants, with a maximum grant of $2,000,000 per organization during the biennium
