Prohibits new assisted living facilities with 6 or fewer residents from being licensed if located within 1,320 feet of an existing small assisted living facility or residential program serving 6 or fewer residents

Removes existing exceptions that previously allowed residential programs to bypass the 1,320-foot separation requirement, including exceptions for small programs outside first-class cities, foster care, and community residential settings

Requires the commissioner to notify affected municipalities within 30 calendar days after issuing provisional licenses or approving relocations for small assisted living facilities and residential programs

Mandates sober homes notify their municipality within 30 days of beginning operations, relocating, or ceasing operations, with existing sober homes required to report their addresses by December 31, 2025

Effective date is August 1, 2025, applying to licenses issued on or after that date
